New Member Process

Before becoming a member of the HPAI we need to verify a few details.

Please review the Application To Join Information Pack.

Print and complete pages 2 to 13 and provide:

• A copy of your certificate (or certificates, if applying for more than one therapy). You may also include other therapy qualifications providing they are included on our insurance banding list.
• Supply 2-character references, not family or close friends.
• 2 passport size photographs for identification purposes.
